Carboplatin hypersensitivity reactions in Korean patients with gynecologic cancers.

Abstract
Carboplatin has established an important role in many different cancers including gynecologic cancers. Although the reported incidence of carboplatin hypersensitivity is low, its occurrence is important to be characterized because of potential fatal complications. In addition, previous studies suggest a relative high incidence of carboplatin hypersensitivity reactions in gynecologic cancer patients compared to total cancer patients. The purpose of this study was to determine incidence of carboplatin hypersensitivity in Korean gynecologic cancer patients of Asan Medical center and identify symptoms of carboplatin hypersensitivity reactions and potential risk factors that may contribute to development of carboplatin hypersensitivity reactions. This was a retrospective study analyzing electronic medical records (EMR) between January and October 2008. Data included patients¡¯baseline characteristics and detailed information about carboplatin administration. Positive carboplatin hypersensitivity reactions were identified from the documentation provided in the patient EMR. The incidence of carboplatin hypersensitivity in gynecologic cancer patients at our institution was 7.9%. Statistical significant risk factors related to carboplatin hypersensitivity were the history of drug allergies and prior carboplatin exposure. Symptoms of hypersensitivity were chest tightness, itching, facial flushing, palpitation, vomiting, shock (hypotension). Treatment of these reactions is various that included administration of histamine-1 antagonist and steroid, full drip of normal saline, oxygen therapy.
